Issuing a New Passport

Passport Renewal

Please send the following documents to the Libyan Embassy:

Completed application form (please fill out all relevant information)
Completed registration form for Libyans residing abroad
The passport intended for renewal
2 photos
A prepaid envelope with the tracking number and the address to which documents should be returned
Mail all completed forms and required documents to:

Adding Children to Passport

Please send the following documents to the Libyan Embassy:

Original U.S. birth certificate
Family book or original marriage contract
Copy of father’s passport (to add to mother’s passport)
Copy of mother’s passport (to add to father’s passport)
For new blue passports: 2 newborn photos size 2x2 cm
2 completed add forms
1 completed newborn notification form
1 completed newborn registration form
1 completed birth certificate issuing form
1 completed registration of Libyans residing abroad form.

A signed letter of consent by the mother, certified by a notary public to add to the father’s passport.

For non-Libyan wife:

A signed letter of consent by the mother certified by a Notary Public to add to the father’s passport.

Note: Failure to provide small size photos for the newborn will result in rejection.

Obtaining a Travel Document

Please send the following documents to the Libyan Embassy:

Written request
Completed travel document form (find below)
Passport loss incident official report
One of the following: a copy of one's Libyan passport, a copy of one's family book, or the official birth certificate issued by the civil registry in Libya (in the absence of a Libyan passport)
2 photos
A prepaid envelope with the tracking number and the address to which documents should be returned
